This Site is an instrument that allows you to electronically submit referrals, along with patient admissions documentation, to us. Additionally, this Site enables you to easily obtain admission request process tracking and status. PATIENT INFORMATION AND OTHER USER SUBMISSIONS

In order to utilize this Site to submit and track patient referrals, you will be required you to submit certain specified information to us, including (i) information about yourself and the corporation, governmental organization or other legal entity you represent, and (ii) patient information, including protected health information, as such term is defined by the Health Insurance Portability and Accountability Act of 1996, as amended (collectively “User Submissions”). We will, at all times, comply with our applicable legal obligations as they relate to patient information submitted by you and nothing in these Terms of Use or our Website Privacy Policy fmcna.com/privacy-statement/ is intended to diminish our obligations under applicable law. Upon our receipt of User Submissions, we will evaluate our ability to admit the patient based on our patient admission policies and procedures, available resources, medical director clearance, applicable law, and our receipt of all required documentation. User Submissions must comply with the Content Standards contained in these Terms of Use.

We do not claim ownership of any User Submissions. Except as otherwise prohibited by law, by initiating or making any User Submissions, you automatically agree to grant us an unrestricted, irrevocable, worldwide, royalty-free license to use such information to fulfill the purpose for which you provide it. You represent and warrant that you have the right to submit the User Submissions to us. Additionally, you represent and warrant that you have the right to grant the license granted above to us and that all of your User Submissions do and will comply with these Terms of Use.

CONTENT STANDARDS

The content standards set forth in this section (the “Content Standards”) apply to any and all User Submissions, your use of any interactive features of this Site (except and to the extent additional terms and conditions apply to the use of one or more interactive features) and to any links or hyperlinks to this Site or Content as described in the “Linking to this Site” section set forth in Section 8 above. User Submissions must in their entirety comply with all applicable federal, state, local and international laws and regulations. Without limiting the foregoing, User Submissions must not:
